Medication Switches

Nottingham and Nottinghamshire ICB have requested that certain patient medications are switched, the switches will be carried out of the PCN Pharmacy Team on behalf of the practice. The following medications are affected:

Circadin® 2mg modified-release tabletsMelatonin MR 2mg tablets
Maintenance dose colecalciferol including 800 and 1000units (all brands other than Valupak and generic prescribing). No change to treatment doses.ValuPak® vitamin D food supplement
